Register for Sports

All students wishing to participate in athletics at Memorial Junior School are required to obtain a physical from their healthcare provider. The physical examination must take place within 365 days of the first practice.

Step 1: Obtain a Physical

A current and valid physical is one performed by a healthcare provider and cleared by the school physician before a student participates in tryouts, practices, or games.

The school physician will review the Medical Eligibility Form upon receipt. The school district will then provide the parent or guardian with written notification signed by the school physician stating whether participation in school athletics is approved or not approved.

Step 2: Register for Sports

A parent or guardian and student-athlete must complete the Arbiter Sports registration process. Arbiter Sports was formerly known as FamilyID.

Medical Plans and Additional Requirements

Asthma: If your student-athlete has asthma, they will not be eligible to participate in sports until a signed Asthma Treatment Plan has been received from your private physician.

If your student-athlete has been symptom free for an extended period of time and does not require the use of an inhaler, a physician statement indicating that an inhaler is no longer necessary must be submitted.

Life-Threatening Allergies: If your student-athlete has a life-threatening allergy that requires an Epi-Pen, they will not be eligible to participate in sports until the signed administration of epinephrine forms are received from your private physician.

Arbiter Registration Account Setup

1. Click the blue Create Account button in the top right-hand corner.
2. Create your secure Arbiter Registration account by entering your name, email address twice, password twice, and checking the I agree to the Arbiter Registration terms of service box.
3. Select the green Create Account button.
4. You will receive an email with a link to activate your new account. If you do not see the email, check your spam or junk folders.
5. Click the link to activate your account.
6. This will bring you to the Arbiter Registration dashboard.

Registering for a Memorial Junior School Sport

1. After creating an Arbiter Registration account and verifying your email, or after logging in as an existing user, you will be brought to your dashboard.
2. Type Memorial Junior School in the search box.
3. Find the Memorial Junior School Athletic Program and select the blue link with the program title to open the athletic registration page.
4. The program landing page will show a description of the program with a green Register Now button. You may also scroll down until you see the blue header labeled Selections to begin registration.
5. At any time, you may log in to www.familyid.com to update your information and check your registration.
